STUDENT ENROLLMENT REQUIREMENTS

 

All students new to the district and former students who have withdrawn from the district must complete enrollment at the Enrollment Center . Student must be residents of the Wagoner Public Schools District, living with a parent or legal guardian.

ITEMS TO BRING WITH YOU:

CUSTODY PAPERS – Only custodial guardian may enroll student. PROOF OF RESIDENCE is required.

Acceptable proof of residence includes current, original electric, gas or

water bill reflecting the service address OR current, original lease

agreement/house contract reflecting property address, agent’s name,

address and phone number. The district will not accept telephone or

cable bills, driver’s licenses, check or pieces of mail as proof of residence.

 

IMMUNIZATION RECORDS are required.

All series must be either complete or up-to-date.

A CERTIFIED BIRTH CERTIFICATE - All students need a birth certificate to enroll. Preschool students must be four on or before September 1 st and Kindergarteners must be five on or before September 1 st.

A withdrawal form and grades are required for all secondary students.

An unofficial transcript is required for 9 th – 12 th grade.

An IEP is required for Special Ed students.
